The Health Post Scorecard initiative by SNEHA focuses on strengthening maternal and newborn care at the primary level through urban health posts in Maharashtra. Recognizing the gaps in antenatal and postnatal services at these health posts, the initiative aimed to standardize and improve care delivery. Weekly antenatal clinics were introduced with fixed schedules and core services like weight measurement, blood pressure checks, iron supplementation, and referrals for high-risk cases. The intervention emphasized equipping health posts with essential resources and delegating responsibilities among staff to ensure efficient service provision.

The scorecard approach evaluates the performance of health posts using specific criteria, such as service availability, infrastructure, and staff compliance. SNEHA frontline workers conduct monthly observations to identify gaps and provide feedback to health post staff, fostering collaboration rather than fault-finding. Health exhibitions and feedback meetings between beneficiaries and providers promote community engagement and encourage better utilization of services.

Despite challenges like staff shortages during national campaigns and lack of functioning equipment, the initiative has improved service delivery in over 60% of health posts across seven municipal corporations. Continuous advocacy at both health post and municipal levels has been crucial in addressing infrastructural and operational shortcomings. The program's participatory and solution-oriented approach has facilitated improved access to maternal care for underserved communities.
