Through various focus group discussions, SNEHA identified key challenges faced by CHWs in their daily work, such as difficulties in scheduling visits and following up with community members. Based on these insights, the app was designed to provide features like automated scheduling, reminders for upcoming tasks, and tools to support CHWs in their interactions with low-literacy populations using audio-visual resources. The app was initially piloted in two community centers, where it helped reduce manual planning by generating visit schedules based on program protocols.

The app is integrated with CommCare, another tool used for data collection, allowing seamless data transfer between the two systems. This integration enables field workers to access case information and past visit histories, helping them manage follow-up appointments more effectively. However, some technical challenges were encountered during the pilot, such as syncing issues and difficulties accessing the app, which were resolved through further development and testing.

Overall, the introduction of the TeamCare app is expected to improve the efficiency of SNEHA’s community health interventions by reducing the administrative burden on CHWs and enabling them to focus more on service delivery. The document also outlines plans for further scaling up the app’s usage across more of SNEHA’s programs, following the successful completion of the pilot phase.
